Cognitive Solutions LLC provides assistance in developing dementia friendly and capable hospitals and healthcare facilities. Thinking about Thinking™ is a half day seminar and planning session for selected executive and clinical staff to review the key role that cognition plays in patient success in the acute care environment. During the seminar we will specifically review best practices approaches to caring for patients with cognitive impairment within the emergency room and medical wards and manage transitions in care and reduce readmission risk.

Key material includers an overview of detection, procedures, and facility accommodations for patients with cognitive impairment and their family caregivers in the admissions process, during their stay, through discharge and the development of critical relationships in the community to extend hospital resources and reach in helping this population.

The Hartford Foundation has identified mentation as one of the pillars of an age friendly healthcare entity and this seminar supports both the concepts and practical aspects of dementia care.

Each participant in the seminar will receive a reference guide of key information from journal and public domain sources.
